One of those is the decision of whether or not to cure the Genophage and, regardless of the choice, it's one with lasting repercussions.
This all goes down in the mission Priority: Tuchanka, which confirms that while Eve is the key to curing the Genophage, creating one will take time.
This means players must defend themselves and Eve from the Reapers while transporting Eve to Tunchanka safely. Quite a few side missions are made unavailable following this one, so getting those out of the way would be optimal, especially to ensure all Mass Effect 3 endings are accessible. There's no right or wrong answer as to whether the Genophage should be cured. However, Wrex or Wreav is convinced that curing it is required to complete the treaty between the krogan and the turians, and Shepard learns early on that the Shroud was sabotaged to prevent any dispersal of the cure. This means that Shepard is ultimately left with the decision to allow Mordin Solus or Padok Wiks to stabilize it, or stop them and allow the cure to go awry.
There are all sorts of outcomes as the result of this final choice and a few others made during the same mission, though stick around. Notably, all outcomes are available regardless of whether Mordin or Wiks are present, making their names interchangeable. After some time passes after rescuing Eve from the Cerberus attack on Sur'Kesh, Mordin will propose using the Shroud on Tuchanka to dispense the cure. Once you initiate the mission, the Salarian dalatrass will tell Shepard that the Shroud was sabotaged in the past to prevent a cure from being released.
She will also offer Shepard an ultimatum. If Shepard sabotages the cure, then she will give them Salarian support and war assets in the war against the Reapers. If Shepard does not sabotage the cure, then she will not give them those assets. You will be facing Reaper enemies during the course of this mission. They primarily consist of husks, cannibals, marauders, brutes, and ravagers.
It is best to have a good balance between combat, tech, and biotics-focused squadmates. The mission will open with the clan leader, Wrex or Wreav, talking about how this will be a defining moment for the Krogan. Eve will notice that Shepard looks conflicted and ask them what's wrong. Here you have the choice to reveal the shroud sabotage and dalatrass deal. But before you can, you are interrupted and have to deal with some Reapers.
After that some krogan will be on the verge of fighting each other before Eve interrupts and gives a rousing speech to inspire the krogan. After this, the Krogan convoy heads to the Shroud. If you chose to try and reveal the shroud sabotage, then Eve will ask what you were trying to say.
Here you can finally reveal the dalatrass deal uninterrupted. It is important to note that if you wish to sabotage the cure, you should not reveal this. Eventually, the convoy will stop at a collapsed bridge, which delays your arrival. Possibly with the assistance of Binary Helix or even Sovereign , Saren Arterius develops a genophage cure and uses it to breed a krogan army on Virmire. Shepard and Captain Kirrahe 's determination to destroy Saren's base, and thus the cure with it, can cause a rift between the Commander and Wrex if he was recruited to the team.
The exact nature of the cure is never discovered and it is lost when Shepard and Kirrahe nuke the facility. Saren's base on Virmire is referred to as a cloning facility several times. How Cerberus learned this is unknown, but it would explain the reproduction of krogan without "curing" them.
Commander Shepard can speak to Mordin Solus about the genophage. According to Mordin, the krogan were beginning to evolve a resistance to the genophage, allowing the rate of viable pregnancies to increase significantly. Mordin explains that he and his colleagues worked on a new genophage variant: in order to keep the krogan population from rising, population growth was adjusted to pre-industrial levels.
During Mordin's loyalty mission, Shepard helps him find his former student Maelon , who had been kidnapped by krogan Blood Pack mercenaries on Tuchanka. When they arrive at the base where he is kept, it is discovered that the Blood Pack is using Maelon to develop a cure for the genophage in order to strengthen their group.
Several dead test subjects are found, including several humans for their genetic diversity and a female krogan. This horrific sight causes Mordin to reveal to Shepard that even though he felt the genophage was necessary, he felt guilty about his work on it. Eventually, Shepard and Mordin find Maelon, who reveals that he was working on a cure on his own free will and feels no guilt for the people he killed in order to find it.
After dealing with Maelon, Shepard has a choice to destroy Maelon's data, or to keep it. It is revealed that some krogan females managed to survive Maelon's experiments.
